Please have the following ready for your appointment:
- A referral from your general dentist
- Current dental images if available
- A list of medications that you are currently taking
- Your medical and dental insurance information
- Valid identification, such as a driver’s license
- Please complete your patient registration forms online ahead of time. If not completed before the appointment, please arrive early to complete them in office. Help from our staff will be available if needed.
